The Worcester Cold Storage and Warehouse Co. fire began on December 3, 1999, in an abandoned building at 266 Franklin Street, Worcester, Massachusetts, USA.
The fire started accidentally sometime between 4:30 and 5:45 PM by two homeless people who were squatting in the building and had knocked over a candle. They left the scene without reporting the fire.

Oxnard Fire Captain Scott Carroll passed away suddenly and tragically from a medical emergency in the evening hours of November 30th, 2015. Reportedly it was tragically an aneurysm that took Brother Scott from us..
During Scott’s 19 year career he held the positions of Firefighter and Fire Captain. Scott was an active member of the Oxnard Fire Department and was involved with training Probationary Firefighters and current personnel in Wildland Firefighting and Hazmat.
His contributions to the Oxnard Fire Department go far beyond his day to day duties as Fire Captain and he will be remembered as a great friend, mentor, and brother to all.
Last night word was received that the medical examiner was releasing Scott to his family. A plan was made for on-duty crews to help escort Scott to his city home of Camarillo.
This morning the Carroll Family conveyed to the Department that they also wanted to go to the Medical Examiner’s office to accompany Scott home.
Some calls were made and the impromptu escort ceremony was put together. The local departments organized the on duty companies to line the bridges to honor Scott going home.
TK-61 and E-64’s crews performed the escort detail flawlessly showing dignity to Scott.
Scott’s family, with tears running down their faces said how they deeply appreciate the escort ceremony.
This is something Scott’s family needed to see and be a part of.
This small act of our tradition will stay with the Carroll family the remainder of their lives.
It was a very proud moment for the Oxnard Fire Department.
A sad, but also proud moment for the Brotherhood.
Please keep Scott's family in your hearts, thoughts and prayers.

The fire started accidentally sometime between 4:30 and 5:45 PM by two homeless people who were squatting in the building and had knocked over a candle. They left the scene without reporting the fire.
The old 6-story building, previously used as a meat cold storage facility, had no windows above the ground floor and no fire detection or suppression systems.
The fire, which started on the second story, burned undetected for 30–90 minutes.
An off-duty police officer first called in the fire at 18:13 after noticing grey/white smoke coming from the roof of the building.
At around the same time an off-duty firefighter from neighboring Auburn passed the building and radioed his Fire Control to report smoke coming from the roof. He told them to inform the Fire Chief "this is going to be a multiple-alarm fire."
The owner of a neighboring business informed a police officer at the scene that a homeless couple had been squatting in the building.
The Firefighters initiated a search, believing they could still be trapped inside.
The firefighters went into search and rescue mode... but tragically the conditions inside the building deteriorated rapidly.
To start, the Firefighters were unfamiliar with the very confusing layout of the building, and most of the floors inside - each up to 15,000 square feet - were divided into a labyrinthine maze of connecting meat lockers.
The walls and many ceilings were covered with insulating layers of cork, tar, expanded polystyrene foam, and spray-applicated polyurethane foam.
There were no fire walls or fire doors, and only a single staircase extended from the basement to the roof.
Worcester Fire Department District Chief Michael McNamee said: "There was a light smoke condition in the upper levels of the building to the point we didn't even have our face pieces on. Within four seconds it went from that condition to the building being filled completely with black, hot, boiling smoke."
The layout of the building and the absence of windows left firefighters without a secondary escape route and prevented ladder and rescue operations.
Six firefighters were still unaccounted for in the building when the interior floors collapsed to the second story level.
They were the city's first firefighting deaths in 36 years.

There is a very dedicated team of advocates currently in Washington making sure we NEVER FORGET and working hard to pass a permanent version of Zadroga .
The "Zadroga Act" established the World Trade Center (WTC) Health Program. This program ensures that those affected by 9/11 continue to receive monitoring and treatment services for 9/11-related health problems.
Some members of that team were in a very important meeting this morning at 10:30 EST.
The meeting took place with 4 members of the team and Senate Majority Leader Mitch McConnell (R).
The meeting has now ended. There are not many comments about what was said or happened IN the meeting, but it does appear, (for now at least)… that we have both his attention and commitment.
The Senate Majority Leader has apparently committed to a permanent health bill for the 9/11 sick and dying first responders.
Although there seems agreement on the compensation portion, there is no agreement on PAYING for the health portion of the bill.
So we can hold calls and emails, for now…
But please understand in the very near future they may need our help again.
Thank You all for making sure our Brotherhood stays STRONG.

Senate Majority Leader Republican Mitch McConnell is practically single-handedly blocking crucial funding for 9/11 survivors.
The Zadroga Act allows funding for the program for 9/11 first responders and other victims of the terror attacks' aftermath.
It officially expired last fall and will run out of money early next year.
Democrats are blaming McConnell for the hold-up, which leaves just one more chance for advocates to pass the legislation before the end of the year.
"It is just plain wrong to play politics with the healthcare of these 9/11 heroes. These brave men and women rushed headfirst into the towers to save the lives of others, which is why this bill has broad bipartisan support, a majority of both the House and the Senate, and needs to be passed right away," New York Sen. Chuck Schumer said.

Judiciary Committee Chairman Bob Goodlatte (R-Va.), (who'd long opposed permanently reauthorizing the program) has come around and told leader on the legislation Rep. Carolyn Maloney (D-N.Y.) he now backs it, so does House Speaker Paul Ryan (R-Wis.).

Millions of dollars were spent, and voluminous reports were generated.
The highly regarded "International Society of Fire Service Instructors" (ISFSI) even published a bold position statement calling for radical change.
The ISFSI position statement encourages fire departments "to adjust their tactical plans and training programs to incorporate NIST & UL research into their emergency response operations."
This was meant to be a "call to action" to springboard research-based intelligence into fire service training and operations.
The problem?
Change is slow to reach the fire drill / training ground and even slower to become department standard operating procedures (SOPs).
The U.S. Fire Service has multiple 100’s of years of tradition. It can be historically traced all the way back to a great conflagration in Boston, MA in 1631.
