From the EMS World Archives: Patients with blast injuries are difficult to manage because their most obvious injuries may not be the most likely to contribute to their mortality. From full-thickness burns to partial and full amputations, blast injury survivors may present with graphic injuries. Nevertheless, death could also be caused by something less obvious, such as a pulmonary contusion, tension pneumothorax or hollow organ rupture produced by the shock wave. www.emsworld.com/10913345

Editorial: "Alexandria Fire Chief Robert Dubé has proposed eliminating the positions of dedicated single-role paramedics, replacing them instead with paramedics who also are trained as firefighters. He also wants to decrease the number of these highly-trained medical providers on ambulances, in order to place them on fire engines. The question that Alexandria residents and city officials must ask is: What is wrong with the city’s current EMS system?"
"The men and women who do this kind of work aren’t your average folks. They need to be able to handle extreme crises, very sick and badly injured people, and they need to keep their cool throughout."
Crews flying those aircraft started their careers as CHP officers on the ground. A helicopter is staffed by two officers doubling as a pilot and a paramedic, while the airplane is staffed by two officers, one a pilot and one designated as a “flight officer.”
"Anne Jensen holds an iPad containing a revelatory program that she helped develop with a local software firm. It is crucial to an expanded campaign by medical, social-services and other leaders in San Diego aiming to help the most frequent users of 911 services. Called Street Sense, it tracks 911 calls throughout the city. Jensen belongs to a six-member team seeking to reduce 911 calls made by frequent users — mostly homeless men and women who would be better served with coordinated social services and regular medical care."
Responders “become emotionally attached to an incident and find it difficult to talk about because of that emotion. There is a guilt that they could have done more and they didn’t do enough and they internalise it and become distant, morbid and inwards."
"We have lost medics throughout the years because they have the courage to go forward and rescue their comrades under fire," said Maj. Gen. Steve Jones, commander of the Army Medical Department Center and School and chief of the Medical Corps. "With the newer technology, with the robotic vehicles we are using even today to examine and to detonate IEDs [improvised explosive devices], those same vehicles can go forward and retrieve casualties."
