The NIOSH Firefighter Fatality Investigation and Prevention Program has released the line of duty death report of Cincinnati FAO Daryl Gordon who died after falling down an elevator shaft on March 26, 2015.

The report provides the details of what happened and gives us several contributing factors. Among them are a breakdown in crew integrity; unsecured and unguarded elevator hoistway door; poor visibility due to moderate smoke conditions; and a delay in getting water onto the fire burning in a second-floor apartment.
